Scheduling an Appointment

Appointments and Registration

Appointments for all our office locations can be made by calling our main telephone number, 317.926.1056 or 800.283.1056. The scheduling staff will work with you to find an appointment time and office location that will meet your needs. If you feel your medical condition requires special or immediate attention, please let us know so we can get a member of our nursing staff involved in scheduling an appointment for you as soon as possible. Our phones are answered during normal office hours which are 8:00 a.m. to 5:00 p.m. Monday through Friday. After hours, calls are directed to our answering service.

Your First Visit

Please arrive 20-30 minutes early for your first visit to our practice. There are several forms to complete prior to seeing the doctor, and they can take awhile depending upon your medical history. If you would like to save time, click on the New Patient Forms in the sidebar to download, print, and complete these forms prior to coming in for your appointment.

As part of your visit, please bring the following:

  • Medical insurance card(s)
  • Drivers license or other photo identification
  • List of current medications (prescribed and over the counter)
  • List of known allergies
  • Recent test results, including CT scan films, applicable to the reason for your visit
  • Referral form, if required by your insurance

If you have a co-pay with your insurance, we will collect it prior to you seeing the physician.

Office Schedule

We do our best to adhere to the appointment schedule. We appreciate the value of your time and share your dislike for long waiting times. Unfortunately, we sometimes run behind schedule. If this happens when you're scheduled to be seen, we will offer you the option to wait, return for a later appointment that same day if one is available, or reschedule your appointment for another day.

During Your Visit

During your visit, your provider may give you prescriptions, instructions, or other recommendations. We encourage you to ask questions. If your provider has indicated the need for a follow up appointment, we can schedule it when you check out or you can choose to call us to make the appointment.

Cancelling or Rescheduling Appointments

If you need to cancel or reschedule your appointment, please provide us with at least 24 hours notice so we have an opportunity to offer the appointment time to another patient.

Telephone Calls

When you call our office, we gather as much information as possible so your questions can be addressed and answered effectively and efficiently. If you're calling for a prescription refill, have questions regarding a recent or upcoming surgery, or have questions that are clinical in nature, your call will be directed to your provider's nurse or medical assistant. If the nurse or medical assistant is unavailable, you may be asked to leave a voice mail message. Your call will be returned as soon as possible.

When calling for a prescription refill, please call during office hours when your medical records are readily available. In some cases, the doctor may want to see you in the office before refilling the prescription. Only prescriptions for emergency conditions are normally provided after regular office hours.

Urgent or emergency calls are accepted at all hours. In the case of an emergency after hours, our answering service will answer your call and notify the doctor on call.
